Clear Aligners Cost: U.S. vs Canada vs Colombia — Why Orthodontics Is Not a One-Trip Product

An aligner quote buys a supervised program that runs for months and ends with retainers. Compare countries on who supervises it, not just on the price of the trays.

The short answer

Compare the whole program, not the price of the trays: records and exam, treatment design, the aligners, attachments and any enamel reshaping, monitoring visits, refinements and retainers. Across borders, the deciding question is who supervises the tooth movement month by month, and what happens when the teeth stop following the plan.

You are buying supervision, not plastic

An aligner case is a prescription treatment that runs for months. The ADA's MouthHealthy guide to braces (2026) describes removable aligners as an option used under the direct supervision of a dentist or orthodontist. It says most people are in orthodontic treatment for one to three years, followed by a retainer phase that holds the teeth in their new positions. That timeline is why aligners don't fit the one-trip model that works for some other dental work abroad.

Regulators have recently underlined the supervision point. ADA News reported in October 2026 that the FDA warned consumers about aligners sold directly without a dentist's or orthodontist's oversight. The FDA's position is that clear aligners are prescription devices. It says it has not authorized aligners sold to consumers without a prescription and professional supervision. It reported cases of new or worse tooth and jaw pain, bite problems, worsening misalignment and new gaps between teeth. It also noted that “FDA registered” only means a facility and its products are listed with the agency. It does not mean a product has been reviewed and cleared. The FDA recommends an evaluation before starting, preferably in person, and periodic in-person visits during treatment.

What an aligner quote should list

LineWhat to askWhy it matters
RecordsDoes the fee include the exam, X-rays and the digital scan?Tooth movement should start from a checked baseline, not from photos
Treatment designWho designs and approves the plan: a dentist, an orthodontist, or a lab technician? The clinician who approves the movement is responsible for it
AlignersWhich system, and roughly how many stages? The stage count is a rough guide to how long treatment will take
Attachments and reshapingAre bonded attachments or interproximal reduction planned? Both happen in the chair, so the visits have to happen somewhere
MonitoringHow many check-ups, and are they in person or remote? This is the line most likely to fail across borders
RefinementsHow many extra rounds are included, and for how long?Teeth often need further aligners when they don't track fully
Off-track planWhat happens if the teeth stop following the plan?Rescans, new aligners or a switch to braces can all add cost
RetainersHow many sets are included, and what does a replacement cost?Retention lasts far longer than the active treatment
Small emergenciesWho fixes a lost aligner or a broken attachment?Small problems far from the clinic can cost a trip

What moves the price of an aligner case

Before comparing countries, it helps to know why two aligner quotes in the same city can differ. The main drivers are clinical and structural, not geographic:

If one quote assumes a simple case and another assumes a complex one, the price gap tells you about the diagnosis, not the country. Ask each provider to describe the case in a sentence, for example “mild upper and lower crowding, no bite change,” and check the descriptions match.

How the three countries differ

United States

Aligner treatment is offered by general dentists and orthodontists, alongside direct-to-consumer sellers that the FDA warning now addresses. If you have dental insurance, check whether your plan has a separate orthodontic benefit, whether adults qualify, and how it pays out over the course of treatment. Don't assume your general dental annual maximum covers orthodontics.

Canada

The Government of Canada's CDCP coverage page (2026) says orthodontic services are currently not available under the plan. Eligible Canadians shouldn't expect the CDCP to reduce an aligner quote at home. That makes the comparison a straight cash-pay one, with travel added on one side.

Colombia

Here the issue is less the price than the structure. Starting aligners during a visit means the exam, the scan, any attachments and the first trays happen in Colombia. Then months of movement have to be monitored from a distance or by someone at home. Before comparing price, ask the clinic how it supervises international patients and what it does when a case goes off track after you have left.

Three ways to structure a cross-border aligner case

  1. Start and finish in the same place. This suits people living in Colombia for a long stretch, or residents. The supervision problem disappears, and the comparison is close to a local one.
  2. Start abroad, monitor remotely, return for refinements. Count the return trips the clinic expects, and ask what triggers an extra one. Remote check-ups may be useful, but the FDA notes that in-person care lowers the risk of complications.
  3. Start abroad, hand over to a provider at home. Check before you travel whether a local dentist or orthodontist will take over a case designed elsewhere, and on what terms. Some may want to re-examine and re-plan, which you would pay for.

An illustrative comparison

The numbers below are invented to show the method. They are not real prices.

Quote A, at home, is $5,500 and includes records, aligners, two refinements, monitoring visits and one set of retainers. Quote B, abroad, is $3,000 for records, aligners and attachments. Refinements are charged separately and need an in-person rescan. If B needs two trips over the course of treatment, at an estimated $1,600 each, plus $300 for retainers ordered afterwards, it comes to $6,500. That is more than A, before counting time off work. If you were going to make those trips anyway, the picture changes. The break-even guide shows how to share travel costs fairly when a trip has more than one purpose.

The retainer line nobody budgets for

Teeth tend to drift after orthodontic treatment, which is why the retainer phase exists. Ask how many retainers come with the quote, whether replacements can be made from a stored digital scan, and whether a clinic abroad can ship them to you. Over the years, replacement retainers and the occasional check can add up to a meaningful share of the original price. They belong in the comparison.

Red flags specific to aligners abroad

Revision terms matter here as much as in restorative work. Ask what the clinic will redo, and at whose cost, if the result doesn't match the approved plan. The remake and revision policy guide lists the questions to put in writing.

FAQ

Can I get clear aligners in Colombia in a single trip?

You can start treatment in one trip, with the exam, scan, attachments and first aligners. Orthodontic movement then continues for months, so the quote should explain how monitoring, refinements and retainers will work once you are home.

Does the Canadian Dental Care Plan cover clear aligners?

The Government of Canada’s CDCP coverage page (2026) says orthodontic services are currently not available under the plan, so an aligner quote should be treated as cash-pay.

Are mail-order aligners FDA authorized?

According to ADA News (October 2026), the FDA says it has not authorized aligners sold directly to consumers without a prescription and professional oversight, and that “FDA registered” does not mean a product was reviewed. You can ask your provider whether a product is authorized, or search the FDA 510(k) database under product code NXC.

What is an aligner refinement?

A refinement is an extra set of aligners made from a new scan when the teeth have not moved exactly as planned. Ask how many refinements the quote includes, for how long, and whether the rescan has to happen in person at the clinic.

How long will I need to wear retainers?

The ADA’s MouthHealthy page describes a retainer phase that holds teeth in their new positions but does not set one duration for everyone. Ask your provider for the retention plan and the cost of replacement retainers before comparing quotes.
